Urethral Bulking for Incontinence – Post-Procedure Care

What is Urethral Bulking?

Urethral bulking is a minimally invasive procedure used to treat stress urinary incontinence (SUI), which occurs when urine leaks during activities like coughing, sneezing, laughing, or exercising. The procedure involves injecting a biocompatible bulking agent into the urethral walls to improve closure and reduce urine leakage.

Urethral bulking is often recommended for patients who:

  • Have mild to moderate stress incontinence.
  • Prefer a non-surgical alternative to sling surgery.
  • Have not responded well to pelvic floor exercises or medications.

The procedure is performed under local anesthesia or mild sedation and is typically done as an outpatient procedure.

What to Expect After Urethral Bulking

  • Mild burning or discomfort when urinating for 24–48 hours.
  • A temporary increase in urinary urgency or frequency.
  • Mild swelling of the urethral area, which should improve within a few days.
  • A slight improvement in continence immediately, but full effects may take a few weeks as the bulking agent settles.
  • Minimal downtime, allowing for a quick return to normal activities.

Post-Procedure Recovery Tips

  • Drink at least 2–3 litres of water per day to keep the urinary tract clear.
  • Avoid straining during urination; let urine flow naturally without excessive pushing.
  • Avoid caffeine, alcohol, and spicy foods, as they can irritate the bladder.
  • Perform gentle pelvic floor exercises (if advised by your doctor) to enhance continence support.
  • Use a sanitary pad or liner in case of mild leakage while the treatment settles.
  • Take mild pain relievers like paracetamol, if needed.

What to Avoid

  • Avoid heavy lifting and high-impact activities for at least one week.
  • Do not engage in sexual activity for one to two weeks to allow the urethra to heal.
  • Avoid hot baths, swimming, or using a jacuzzi for at least one week to reduce the risk of infection.
  • Do not delay urination; go when you feel the urge to prevent bladder overfilling.

When to Contact the Doctor

  • Severe pain, burning, or difficulty urinating that does not improve.
  • Fever above 38°C, chills, or signs of infection such as foul-smelling urine.
  • New or worsening incontinence rather than improvement.
  • Blood in the urine lasting longer than 48 hours.

Recovery Time

  • Most patients can return to light activities within 24 hours.
  • Full recovery takes one to two weeks, depending on individual response.
  • Some patients may require repeat injections to maintain long-term effectiveness.
